Please complete the enclosed lodging and meals reservation form to arrange for lodging and meals at Breezy Point Resort and Conference Center. Please keep a copy for future reference. Complete one form per person; married or partnered couples and families may complete one form only. Phone reservations will NOT be accepted.
All package rates are per person and include meals and breaks, lodging, use of meeting rooms, recreation areas, service charge, and state sales tax.
PACKAGE ONE – Per person
Lodging: 2 nights (Wednesday and Thursday)
Meals: Wednesday evening reception; Thursday breakfast through Friday lunch, plus refreshment breaks

PACKAGE TWO – Per person
Lodging: 1 night (Wednesday)
Meals: Wednesday evening reception; Thursday breakfast, lunch, plus refreshment breaks

Meals: Thursday lunch, dinner; Friday breakfast, lunch, plus refreshment breaks

INDIVIDUAL MEALS ONLY – If you are attending the conference for the day only, or if you have guests, meal tickets are available for purchase. Meal tickets must be pre-paid. Complete the “meal only” section of the reservation form.
ROOMMATES – If you wish to room with a specific person, please write that person’s name on the roommate section of the reservation form. BOTH people must fill out separate registration forms and submit them together to Breezy Point Resort.
PREFERRED LODGING – Preferred lodging will be held until September 1, 2008. Full package pre-payment is required with your lodging reservation. Make your lodging and meal reservations by fax or mail DIRECTLY to Breezy Point Resort and Conference Center using the enclosed lodging and meal reservation form. No phone reservations accepted. Each room will receive one confirmation.
CHECK-IN TIME – While you are welcome to arrive early to attend meetings or enjoy the facilities, STANDARD CHECK-IN TIME IS 5:00 P.M. on the day of your scheduled arrival. Checkout time is NOON on the day of your departure.
PETS – Breezy Point Resort has a NO PET POLICY. Prior notification of certified service animals is required.
