COMMUNITY HEALTH AWARDS AND RECEPTION

The Community Health Awards recognize and honor local and state public health accomplishments, staff, and elected officials. The awards will be presented by

Sanne Magnan, M.D., Ph.D., Commissioner of Health during the 2009 Community Health Conference, the evening of Thursday, October 1, 2009 at 6 p.m.

The awards presented will be:

Award for Outstanding Dedication to Local Public Health
Commissioner’s Award for Distinguished Service in Community Health Services
Jack Korlath Partnership Award
Jim Parker Leadership Award
Community Health Certificates of Recognition
